The two-page Titanic PDF below shows a 15-Why Cause Map. Page 1 shows how the analysis can start with a basic 5-Why – any of the 5-Whys. As information is collected the analysis expands into more detail without contradicting the different pieces people provide. The 15-Why Cause Map on Page 2 shows a different highlighted color for each of the original 5-Whys.
The Cause Mapping method shifts the discussion from opinion-based “who’s right,” to an evidence-based “what is accurate.” Miscommunications occur when people apply “right-answer” thinking to a problem that needs to be broken down into parts.
